Violence On Television And Children

Who is responsible for film and television ratings for graphic violence and other mature content?
Who decides what is acceptable for viewers of the network? There was a radical change in the type of violence that is now allowed a few years ago. I wonder who did the TV ratings and movies as well. Who decides whether a film should be rated R or another. So why children are allowed into R rated movies with parents, and why they are often video clips in theater management that plays the movie? I know a lot of questions, but they are linked.
It seems that the Motion Picture Association of America decide qualification for the movies (see web link) FCC and control of TV ratings (see link again).
